'No effective follow up': NT coroner recommends sweeping changes after death of teenage boy

A teenage boy who died in a remote Aboriginal community in 2020 "was not afforded basic mental health care" and was wrongly "screened out" by Territory Families, the Northern Territory acting coroner has found.
